13. FitMove Thrive Club

13.1 About the class

Thrive Club is a free, physiotherapist-led community exercise session, run by HJW Therapy Ltd, trading as Home Links Physiotherapy Croydon & Sutton, in partnership with Walfinch Home Care. Sessions take place at Banstead Community Centre. Our physiotherapists are registered with the Health and Care Professions Council (HCPC).

13.2 Medical questionnaire

Before your first session, you must complete our online medical questionnaire, using the QR code or our website. Please answer it honestly and in full, as this helps our physiotherapists keep you safe. We cannot accept responsibility for problems arising from information that was not shared with us. If your answers show you may need extra support, a physiotherapist will speak to you before you take part.

13.3 Yearly update

Your questionnaire is valid for 12 months. We will ask you to complete a new one each year. Please tell us straight away if your health changes, for example a new diagnosis, surgery, a hospital stay or a change in medication.

13.4 Taking part safely

Please work at a level that feels comfortable for you, and tell the physiotherapist if you feel unwell or have pain during a session. Our physiotherapists may ask you to pause or adjust an exercise, or to check with your GP before returning, if they have any concerns about your safety.

13.5 Sharing with Walfinch

With your consent, we share the health, attendance and contact details Walfinch needs to help run Thrive Club safely. You can withdraw this consent at any time by contacting us.

13.6 Photos and videos

We may take photos or short videos at Thrive Club to share what we do, with Walfinch. We will only include you if you have agreed, and you can change your mind at any time.

13.7 Changes to sessions

We may occasionally need to change or cancel a session, for example if the venue is unavailable. We will let you know as early as we can.
